Responsibilities for Case Manager:
- Provide case management services that assist participants and families in gaining (maintaining) access to needed Waiver and other Medicaid State Plan services, as well as medical, social, educational and other services, regardless of the funding source for the services.
- Responsible for day to day oversight and implementation of the Provider Signature Page and for ensuring participant health, safety and welfare
- Ensuring the completion of service agreements or service authorizations between the participant and other service providers.
- Working with the fiscal employer agency to monitor the expenditure of funds according to the individual budget, service plan and service agreements/authorization.
• Working with the fiscal employer agency to determine that domestic employee providers of services, such as Personal Support and Non-Medical Transportation, are qualified and competent to provide the specific services the participant is receiving. - Keep necessary records that are complete, concise, accurate and maintained in a timely, organized fashion in accordance with the requirements of the Bureau of Quality Management
- Make any changes/corrections in service provision as noted in a Division of Developmental Disabilities Corrective Action Plan Maintain QIDP status by completing 12 CEU training credits annually
- Working on a committee or program as assigned.
- Must meet the minimum federal educational requirements for a QIDP outlined in 42 CFR 483.439. Must be willing to attain certification within 6 months of hire.
- Must have at least one year’s experience working directly with people with intellectual/developmental disabilities.
- *Bachelor's degree required*
- *No On Call Hours for this position*
- Ability to coordinate all requirements of the Home and Community Based Waiver program
- Ability to counsel patients and/or families in life management and coping skills
- Ability to gather data, compile information, and prepare reports
- Interviewing and psychological/developmental evaluation skills
- Knowledge of community support services and agencies
- Knowledge of psychosocial interviewing and counseling techniques
- Records maintenance skills
- Skill in the use of personal computers and related software applications
- Respect for and understanding of individuals with disabilities and their right to control their lives while receiving needed supports to reach personal goals
- Knowledge of privacy and confidentiality requirements of persons served
- Ability to work as a team member, accept direction and achieve consensus among stakeholders
- Must successfully complete all trainings required by the Illinois Department of Human Services, Division of Developmental Disabilities to be enrolled in the QIDP database and maintain QIDP status
- Pass all background checks required by the Illinois Department of Human Services, Division of Developmental Disabilities
